Confused - MATB1 Form

Ok, stupid question... but is it my due date on the MATB1 form? xx

Different midwifes seem to fill them in differently some use your actual due date and others use your expected week of childbirth date which commences the Sunday before your due date. My dd is 14.01.12 but the date on my matb1 says 08.01.12 as I can expect to have my baby during this week x
