My wife and I have trying to conceive for about 5 years, and recently it happened. Now I know for a fact that this conception happened on december 8 - december 13 depending on sperm lifespan. We only had sex once in the month of december, and her last period was november 24th.

By my calculation, that puts a conception date at the latest of 4 weeks and 6 days today.

We have had 4 endovaginal scans, and all of them have an empty gestational sac. My position is its too early to tell by scan, but the nurses at the hospital have already declared it a missed miscarriage. I think they are wrong, and heres why:

1. we originally had blood taken (i cant remember what date) and the hcg levels were around 4,600. They had risen 75.6% from the previous 48 hours.

2. She had blood taken on the 17th, and that was around 5,200 hcg. So they had not risen alot, but they HAD risen.

3. the Mean Sac diameter at our last scan was 8mm on the 16th, or around that, but our previous one was 7mm, 10 days before. So not alot of growth there, but it DID grow.

4. If i am correct at 4 weeks 6 days conception date, are we still too early to see anything? We have another scan on the 23rd.

I never give up, and while not perfect, to me the signs still show hope. I however, do not want to give my wife false hope, so im looking for advice on if im headed in the right direction hope wise, or im just not getting the message. Thank you.

I really don’t want to be the bearer of bad news but it’s best to prepare for the worst.
What you are describing is basically what happened to me. I was roughly 4 weeks from conception and the sac was empty. Initially the hcg was climbing then would not double after 48 hours. Was creeping up. With hcg it’s not the numbers that are important but how they look after 48 hours. Some people’s don’t double and go on to have normally pregnancies. For me the pregnancy was failing.
At the next scan they will see if anything is there. Ideally should be at least 10 days from last scan to see if there is growth.
I really hope that the news is good. I know exactly the limbo hour in and I send my hope and wishes to you both. Just be there for each other.
There’s no harm in being hopeful. Anyone in this situation will have a small amount of hope. Please if you can update on how things go. Any questions please ask.

If she had her last period on the 24th, she would be around 8 weeks pregnant (6 weeks is the fetal age, so 6 weeks since conception). But I don't know whether you spouse has long or short cycles.
Have you seen any sign of life or just a sac?
Has she bled at all?
Have you had any blood tests done recently?x

Just the gest sac. She does however, apparently, have a tilted uterus, so im wondering if they are only seeing half the sac. Light spotting maybe 3 times with wiping after pee. like, less than a pinhead size of blood. The last blood test was on the 17th, at 5000 hcg or so.

I really feel for you being in this limbo, I think if the date of conception was between 8th-13th Dec a fetus and heartbeat should be seen by now, she should be at least 7 weeks pregnant (from date of last period which is how pregnancies are dated), but I don’t know when the last scan was when only the sac was seen, if the next scan still doesn’t show a fetus and heartbeat I really would say it’s bad news, slow rising hcg is usually not a good sign in the vast majority of cases either, sorry to be all doom and gloom I wish I could say everything sounded good but i think that would be giving false hope, I’ve been there before when a sonographer tried to give me false hope and i think it’s totally the wrong approach to take, I will keep my fingers crossed for you though that it’s just down to other factors like the tilted uterus or baby is just hiding x

I also have a tilted uterus and had a scan at 6 weeks, we just saw a dot with a flickering heart. She said it was due to the angle of the uterus that we barely could see it.
I don't want to give you false hope because the burden will only be heavier to bear if you think you have lost something. On the 23rd she should be around 8 weeks and a half so if there really is something in the sac, it should be visible for sure!x

You are right that it could be too early to tell right now, so if there is no bleeding rescan in a couple of weeks.
However I have had a miscarriage due to empty sack before (blighted ovum). It was my first pregnancy and apparently it's quite common. We had no idea and I didn't misscarry until 12weeks. The sac itself continued to grow throughout that time without any fetus present. We had a scan at almost 11 weeks which showed the empty sack and then it passed empty when I miss carried naturally.
I know it's very hard, but her becoming pregnant is a good sign (if it is indeed an empty sack), and I myself now have my first son.
Good luck
