hi all,

i was wondering if any of you could help, i have my scan on tuesday (YAY) and just re-read the letter so i could get any paperwork altogether now, and the letter says to bring your wolfson form (lilac and white) apparently the top half should be completed by your midwife (or so the letter says) as i dont have one does anyone know what its for? any help would be nice, thankx xxx

hey hunni. the only lilac and white form i know about is the one they gave when i was discharged from hospital after giving birth to my last son ...... other than that i dont know xx

im going to phone mw in the morning, dont like the feeling of getting there for my scan n being told i cant have it! but only 2 days to wait YAY

I had to take my green notes...never got anything lilac and white...

Just had a look at my scan letter, it says nothing about a wolfson form... i just have to bring my green notes to (plus id)
