NHS and maiden name?

Bit of a daft question but..... I got my 12wk scan appt in the post today and the paperwork is in my maiden name, despite my local doc records being in my married name. Is that normal procedure?
I'd much rather be known by my correct married name.

when you changed your name with your surgery did you get a new NHS card (small and white card with a bit of purple on) stating your local health service and registered practitioner?

i got married in June 09 and changed my name with the doctors & then got that card through in August 09,

i think you also get the cards when you change surgery, well you do in Doncaster :-)

it might be worth mentioning it to your midwife when you see her

Nope, they only change the cards with a change of doctor around here, and as I stayed with the same surgery....
Called to enquire today and it's a simple mix up at the hospital. They say I can sort it when I go for the first scan.

They changed my surname but not my title :wall:


Same as my hospital, they said they can change surname but changing the title is more difficult because the system doesn't like it, but when my mum got married they changed her title and surname which is the same hospital, funny ay ?!? :wall2:
 
I had that but when I went for my booking in appointment with the midwife I told the receptionist and she printed off a load of labels with my new name on that the midwife had to stick over my old name ones!
