I have lean PCOS too so I know what it's like. (Sorry - this is long...)
I have yet to get my BFP but a girl on here, Kitty, did, after 18 months but just one month on metformin. She has a journal here but I don't think she is around much now.
As for me... we have been trying on and off for 7 years. During much of that time, I have experienced pre-menstrual spotting that started off dark but got bright red, heavy, and last two weeks. Some months I was bleeding the whole cycle. My periods were heavy and the cramping was painful. I went to the doctor, they said it was just another period. I went again, they said it could be breakthrough bleeding. In frustration, I mentioned that I knew it was a symptom of cancer so he just got a gynaecologist to check for that then I got discharged. When I started to track my cycles, I noticed that they had also got short (24 days).
About 2 years ago we went back again and asked for a referral to the fertility clinic. They told us to try for 6 months longer but I demanded they check DH was ok (he was) and that I was ovulating (apparently I was). When we went back 6 months later, we got referred to the fertility clinic. At the first scan, they said my ovaries were polycystic looking but it shouldn't stop me conceiving (ha). They said metformin wouldn't work as I wasn't fat. So, they give me clomid in preparation for IUI and I over-respond. I do it naturally three times and all fail. But the progesterone and clomid both helped the spotting. I decided I wanted to sort my body out before IVF.
In frustration, I phoned for an appointment at the clinic in June and was told I actually had typical PCOS (she looked beyond my weight). I felt so happy to have a diagnosis. I Googled supplements and tried cinnamon and chromium. This actually gave me EWCM and made my spotting minimal! I started on metformin 8 weeks ago (I know it DOES work for us lean PCOS girls too), have even more EWCM and a normal AF (three days long, not overly heavy and with no cramps!). I couldn't TTC last cycle as I was preparing for IVF but I feel more optimistic than ever now, and I honestly thought it would never happen. Now I am sure it will. Xxx
