my midwife is giving me the form at my appointment on wednesday x

Your Mw will give you a form, she'll fill in her bit and you have a short bit to fill in

Try and do it quick as they're seriously behind atm Mine has been away for 3 weeks and may take another couple of weeks xx

Of course hun, you just can't apply for it until you're 25 weeks
