Drafted a letter - how does this sound??
I am writing to let you know that I am pregnant and to notify you of when I want to start my maternity leave. I am currently 23 weeks pregnant and my Expected Due date is 9th January 2012. I will forward my MATB1 certificate, signed by my midwife, when I receive it. I plan to start my maternity leave on 4th Jan 2012, which means with Christmas holidays my last working day will be 21st December 2011.
If my leave starts on my intended date, I will return to work on Tuesday 1st May 2012. I also am aware and willing to use my 10 Keep In Touch days, as and when required with prior arrangement with yourself, after my first 6 weeks on maternity, and understand this does not affect my maternity pay allowance and any holidays accrued whilst I am on maternity leave.
