Help please!

Jake is 3 weeks and 2 days and exclusively breast fed. I posted the other day about his naughty new habit of stretching toward the end of a feed, we have a new problem.

Mid feed, he's properly thrashing his head back and for, stretching out, generally flailing around. He doesn't let go of my nipple, but he's pulling it around so much he comes off quite a lot (and we all know how painful it is trying to pull a baby off when he's latched without breaking suction first... It's like that). If I try and wind him, he screams to get back on, and will carry on feeding for another 15-20 mins (occasionally repeating this).


My right nipple has totally recovered from being cracked and broken and had become my "good" nipple - but it's getting sore now for 2 reasons - hour long feeds, and the above messing around. What do I do to make this better????
 
Can you get your mw/hv to observe a feed? I did this in the early days and sorted out all my problems. There are also bf consultants and peer supporters who can help.

Evan used to pull about a lot, and still does sometimes. It's usually when his bottom half is busy lol!
 
Hannah has started this sometimes it's wind, either end. Also sometimes I think maybe milk flow slows so I swap sides and she gets going with gusto. I never know when boob is empty as they never are totally. But I 'think ' my long feeds are because I keep her on one side too long as by the end she's not getting as much. Swapping is helping. I am just guessing though.
 
My girlie does this. It's hard as it bloody hurts!
I struggle as it's for one of two reasons.

She wants more and there's nothing left
Or
She's fed enough but she just wants to comfort suck, and gets frustrated when milk comes out!

Now when she does it and pulls herself off I try and offer a dummy. She's not a fan of dummies, 9 x out of 10 she will spit them out but in this situation eventually she will take it. If she's still rooting with crying I'll give her the other boob.

If only they could talk...
