can i get a matb1 form from a doctor?

i may sound dumb i know i can get one from my midwife but my next appointment is not until 21st dec which is cutting it a bit fine considering i have to hand it in work atleast 8 weeks before i want to go on maternity leave, i havent even decided when to leave yet but i dont want to ring midwife because im sure she has more important things to do then hand out forms xx

Yes doctors can give you a mat b1 form.

I got mine from my consultant at the hospital as my midwife was crap and kept forgetting x

you can try hun but if u have no look try calling ur mw to se what she says but from what my mw said they cant give you one til ur 25 weeks and ur company should no that!! as many women leave at 29weeks so thats only 4 weeks notice
