Can anyone explain my day 21 blood results?

Hi, I am new to this site.

Have been ttc for 20 months with no luck, I have been sent for several day 21 blood tests the doctor says they are inconclusive every time, below are some of my results can anyone explain them to me?
My cycle is usually regular between 24-29 days, but whenever I have day 21 bloods my cycle goes crazy currently day 35 after having bloods on day 21 and 28 this month, not sure if there is a connection.

Serum progesterone 14 nmol/L, progesterone <16 nmol/L

Serum progesterone 1 nmol/L, progesterone <16 nmol/L

Serum prolactin level 491mU/L, Serum total HCG level <1.0iu/L, Serum oestradiol level 165 pmol/L, Serum FSH level 5.5 iu/L

This is three different sets of results.

So I am definitely not an expert. But my understanding was that instead of always being on day 21, it just needs to be done 7dpo. Maybe your results are inconclusive because you have irregular cycles you’re not always 7dpo when you have them done?

It would also be really helpful if your doctor gave you an indication of what a ‘normal’ result is alongside your results.
 
Or is the bit on the right the normal result? So in order to indicate you&#8217;d ovulated they were expecting a result over 16?!
 
The range for progesterone that indicates ovulation is anything from 16 - 30nmol. As your levels are less 16nmol it is SUGGESTIVE that ovulation did not occur. The reason I highlight the word suggestive, is because anything can throw off the results, especially if you have irregular cycles. One of your results is very close to the 16nmol which suggests to me that you came close to ovulating or that you ovulated but the bloods where taken at the wrong time (more likely).

Your best bet is to review your cycles, try to determine a pattern. If you regularly don't have a 28 day cycle then you likely ovulate later in your cycle meaning day 21 bloods will return inconclusive.

But I suspect your GP might want to refer you to a specialist. They'll be able to offer better testing and more information. Make sure you track every cycle. It might help to use opks, it might at least show when your lutenising hormone surges, which is a good indication of when you might be ovulating. This information can than be given to your gp/specialist for better timing of blood testing.

Thanks all, I am usually between 25 - 29 day cycle and tried to explain this to doctor but they just keep saying it had to be day 21. I have also had very long cycles twice when giving blood for day 21 blood tests not sure if this is stress of the test related or just coincidence &#128580;
