Go on the offensive with your food and drink intake. I developed one after a long day when i worked too many hours and stood on my feet all day without a break, and was gutted to say the least. I had been lucky prior to that and in general have had extremely good health during this (first) pregnancy. I was much more depressed by this one (dangly, swollen, itchy) thing than I really should have been, and I am generally not embarrassed by bodily fluid/functions/squeamish type things.
Anyway, I decided to bite back by drinking stacks of water and not letting anything pass my lips that didn't look like it had come straight out of the ground on the end of a leaf or a stalk. So - loads of bean salads, beetroot salads, all the fruit I could get my hands on, cous-cous, all-bran and some fibre drinks and piles and loads of vegetable of any description. I kept this up for a week and the thing shrank to a tiny size and I intend to keep it that way.
The piles are basically little varicous veins in your bum and are caused by a combination of dehydration and the fact that the bump puts pressure on the veins in the area meaning its more difficult for us to squeeze the blood back up towards the heart after gravity has helped it be pumped down to our bums -a price we pay for having evolved off 4 legs and onto 2.
The good news is that it will probably go away altogether after you have had the baby. In the meantime, sleeping on your left hand side also helps takes pressure off the veins, and preparation H helps remove pain/itching and helps shrink it. Apply as often as you go to the loo. (talking of which, try not to "settle in" on the toilet, or strain, as this contributes to the problem. Good luck - and try not to be upset by it. I call mine my bum buddies, in an attempt to make me smile about them.
