I figured I was out this month... has positive OPK on cd5, and all signs/symptoms say I Ov'd on day 6... which is not unusual for me (v short cycles) and I've read most likely leads to an immature egg being released.
But I kept doing OPKs, and got negatives on day 6-10, but then started getting all the usual Ov signs, and hey presto, positive OPK again on day 11!!
I didn't think this was possible, but just found this article:

So turns out it's not that rare after all!
The moral of the story... don't stop 'trying' after you get your first OPK
Praying for a bfp this month and starting my second 2ww lol!!
