maturnity pay form?

was i supposed to be given an maturnity pay form at my 20 weeks scan?

did anyone else get one?

I neer got my blood results or offered to chat throu my questions (and i have loads of them) but i wasn't given my maturnity pay form which apparently should be filled in by the accounts department at work to ensure i get paid when off....

I wasn't given my MATB form until I was 24 weeks - I assumed because thats when the baby reaches viability. It was my Midwife who gave me mine. I believe its either your Midwife or GP who you get this from.

You get the MatB1 form off your midwife around the 20/24 week check up.
