How do I get my MAT1B form, and when do I need to let my emplyer have it?

My midwifes giving me mine at my appointment wednesday...she said they usually give it you after your 20 week scan.

I've had my 20-week scan, and I have to make a GP appt at 25 weeks, and the next Midwife appt is at 28 weeks - so will my GP give it me at 25 weeks?
I'm confused!!!

Its differnent depending on where you are, yes your gp might give it you..if your worried ring the surgery today and ask what they normally do..

My midwife told me I would get mine at my 24 week appointment.

My GP gave it to me when I was 19 weeks. My employer wants to have it before I reach 25 weeks. Make sure you check with your HR so you don't miss deadline for SMP.

I should have got mine at my 24 week appointment but my doc forgot so i rang them and picked it up from the docs reception. Ive already handed mine in to my employers.

I had to ask for mine at my 25 week check up with the midwife. I think I'd wouldn't have got the form if I hadn't remembered to ask for it
