I can't reach either, that's why I get my hubby to do it! The problem is, because I know it's going to hurt, I tense up and then he can't, ahem, insert anything! x
I've used the gel from Mothercare and following what it says on the box you only need to massage it into the area to allow it to soften the skin. And you should do it every day from 34 weeks. However if your feeling brave I guess you could youtube how perineal massage is actually done and give it a go. I certainly couldn't even reach and the other half wasn't too impressed with me asking for his help either! So I've stuck to using the gel and just rubbing it in after a shower...I'm yet to find out whether it has been of use! lol But it's basically olive oil mixed with a bit of candula oil (mothercare one) and I've been told many a times that a bit of Olive oil will work just the same!

I think it's similar to the whole stretch mark prevention theory-you can prep the skin and make it more supple so that stretch marks are less likely, but if the stretching is too exsessive the skin just can't cope, therefore gets damaged. So I suppose you can make the perineal area more stretchy and supple, but it will only cope with a certain amount, and if baby comes out too quick or at a certain angle then it still will tear x x
My second came out that quick mw only just got her gloves on and mats out in time to catch him, I only grazed and didnt do this. My first I was cut and again didnt do it but they cut me to speed up labour as lo heart rate dropped

The thought of tearing terrifies me - might see if I can give this massage stuff a go. If there's even a small chance it might help I'm up for trying at least!
