help writing resignation letter

anyone good at writing letters? i want to say im leaving beacuse of family commitements and because of my back, just dont know how to word it

How about:


This is my letter of resignation and I am writing to give (you/name of company) x weeks notice. My last day of employment will be (date ). I regret that I have to leave the company however due to back problems and family commitments this is the right thing for me to do.
