Does anyone know when we get the card for free prescriptions? I can't remember if we have to request them or if they're just automatically sent?
Thanks
pregnant, or you have had a baby in the last 12 months a maternity exemption certificate or card (Medex). your GP or midwife will give you a FW8 application form. You should send it to your local Health Authority.
go to your GP reception and ask for a maternity exemption certificate... needs authorising by your GP or your midwife.. then send it to address on the document.. should get the card in about 2 weeks
I got a form to sign and post from my mw at my booking appointment. It took about 10 days to come back. I think mine is valid from April 2007 - December 2008. Hope this helps. xxxx
My doctor got me to sign something when I went to confirm pg, I got the card at about 7 weeks. I think that you can reclaim any money you spend as long as you keep a receipt.
