i need to write a letter to my employer telling them that i intend to take the additional 12wks maternity leave, does anyone know of any example letters anywhere?? im rubbish at this kind of stuff

Unless you have written to inform them that you are returning to work early it is assumed that you are returning after 52 weeks maternity leave (unless your work have said otherwise).

Something along the lines of

Dear.......
I am writing to inform you that I intend to return to work on (date).
Yours sincerely,
