Bounty pack received from Midwife

I had my booking appointment on Friday and received a bounty pack from her.

Inside the pack was a few things but also a white piece of paper which said “smoking referral” but next to the questions about whether I smoked all no’s were circled so maybe that’s why it was in the pack because I didn’t need to be referred.

There was also a pale blue card which appeared to be an ultrasound request form at 12 weeks.

Should these two bits of paper be in the pack? I don’t want my scan to be delayed because the card wasn’t sent off.

I can call the midwife to confirm I was just wondering if it was a mistake or not.
 
I didn’t get either of those, just a magazine about breast feeding... best ring to check about the scan. My midwife said I have to take my permission slip with me though otherwise they won’t do the scan - but mine isn’t blue...
 
I would call and ask to speak to the midwife you saw..

As I wouldn't want to her her into trouble
