Maternity Allowance

I was supposed to send off my forms today for maternity allowance, but obviously baby brain has kicked in again + it hasn't been done! LOL..

Just checking what it is I need to send with the forms? I have my MATB1 form and 4 months worth of payslips.. Is there anything else I need to send with my forms?
