MATB1 Form

What on earth is this form and where do I get it from? I have been having a flick through my works maternity leave policy and it mentions that I need this form help?

The midwife will give it to you. In my area you get it at 25wks.

I think it's a form you give to work to confirm you are pregnant so you get mat pay.

you'll get it at 24 weeks ish from midwife its so you can,get mat pay

Basically it just says yes you are pregnant and its given after 24 weeks because that's when your pregnancy becomes viable. X i got mine from my doctor anyone in the medical profession can give it you that is involved in your pregnancy x

i got mine at my 21 week apppintment as my work (NHS) were hassling me for it but you dont need to give it to work until 15 weeks before due date so 25 weeks...youlll get it soon enough x
