I think you get them from the GP surgery - ask youe health visitor about it?

i think its from gp, health visitor or if you have a local community or childrens centre. in my area we have a childrens centre in each town and you apply to sure start there or at the mother n toddler groups they run. health visitors can refer you but i dont know if they will if you dont need the outreach team and you just want to join in there activities. xxxxx

I got my form from the job centre plus office, and there's a section in it that needs filling in by your GP or MW
