Birth Plan

During my 2nd trimester, I printed off an NHS birth plan off their website and filled some of it in straight away. I've since lost it (which doesn't matter because my thoughts have changed now compared to then anyway), so obviously I need to make another.
Just wondering whether to just write it from scratch myself, or get one printed off, which is best do you think?

If I write my own, I would want to make sure I included everything and didn't leave anything out. Anyone wanna give me some ideas, or point me in the direction of a good template?
 
Have you tried searching for birth plan threads in here, some of the girls posted theirs, or templates they were given.

I think some birth plans are a bit much for midwives to read and remember, I would rather write my own one with the most important things at the top of the list than one that goes on and on in sections where things might get missed.
 
My birth plan is all in my pregnancy notes. Went through with midwife at my last appointment.xx
