Birth plan template

For those of you who are writing a birth plan and need a template to get the ideas flowing I found this one before which seems quite good. I printed it off to give me ideas for when I am typing mine up.

That looks quite helpful I can't decide whether to write one or not, I don't really know what I want/don't want aslong as it's whats best for baby xxx
